Team Forms

In order to join and participate on the team, the following must be completed:

Media Consent Form

This form must be signed by a parent or guardian to give permission for their child to be photographed, quoted, filmed, or videotaped for non-profit use (e.g.: educational, public service or health awareness purposes).

Medical Form

A form for medical purposes regarding allergies, vaccinations, etc. Please have your doctor/physician complete this.

You also need to fill out this contact form: 2021-2022 Contact Form. You will also need to request to be added to the Stuyvesant Robotics google group in order to receive weekly emails and updates.

Once you join the team, you must register an account with FIRST: FIRST Student Team Information Members System Form (STIMS)

Team dues are $100 and you will need to pass a safety exam when you join the team.
